"What we are researching is quality improvement and the implementation in a practice of the system that's needed to guarantee reliable, consistent performance," Duffy said. The initiative aims to help between 250 and 300 practices of 10 or fewer clinicians change heart health care while studying the effect of the extensions system. To aid in the process, participating clinics will have access to peer clinician coaches and practice enhancement assistants who will make 32 visits over the course of a year spending at least half a day at a time to help the practices make changes.

Clinics will also be provided assistance in connecting to electronic medical records. "This is a big, multidimensional intervention, and that's why it costs so much," Duffy said. "If we can do this it will truly be a game changer for continuing education and practice improvement. sions created 100 years ago, the total amount of the nation's GDP spent on producing food went from about 45 percent to about 8 percent as new information was able to be channeled through the extension to the farmers. "The thought was, let's do that in medicine," Duffy said.

"Right now we have a medical system that's pretty chaotic with how it distributes information." The idea is that by not only distributing up-to-date information to medical practices across the state, but also helping with implementation and documentation in the offices, positive changes can occur in a number of areas including lowered blood pressure, increased aspirin use for those at risk of heart disease, reduction in tobacco use and lower cholesterol. "All of those four things are known to reduce heart disease, and we need that in Oklahoma in the worst kind of way," Duffy said. Heart disease is the No. 1 cause of death in Oklahoma, and the state has the third -highest death rate from cardiovascular disease in the country, according to the American BY MIKE AVERILL Tulsa World mike.averilltulsaworld.com The groundwork has been laid, and the first of four waves is set to begin in a $15 million plan to improve heart health in communities across Oklahoma and research the results. The University of Oklahoma in Tulsa and Oklahoma City was one of seven grantees to receive federal funding six months ago to participate in the EvidenceNOW Advancing Heart Health in Primary Care initiative, through the federal Agency for Healthcare Research and Quality.

Known locally as Healthy Hearts for Oklahoma, the goal is to model a program based on university agricultural extension programs that allow academic advances to directly help communities. "We need to get that evidence into practices and truly changing people's lives as quickly as we can," said Dr. Dan Duffy, principal investigator with Healthy Hearts for Oklahoma. Memorial services 2 p.m. Saturday, December 24, 2015 In Loving Memory of Diana Miller (52650 122496) My beloved Wife, Mother of our son, Paul, Grandmother of Tyler, Sarah and Katie Mae You must always be so very proud of our son. Paul will soon be opening an eighth restaurant! All the ones he has now are doing quite well. He has even launched a food truck! Paul is working very hard and loving every minute of it. The restaurants continue to receive recognition in the local newspapers and Paul still makes personal appearances when his busy schedule permits.

No doubt, it was your encouragement all those years ago for him to find something he is great at that has led him to these successes. Our son is indeed attaining his life-long dream and goal of becoming a prominent restaurateur. Our grandchildren are growing up before my very eyes. Katie Mae is now 15 months old and walking and talking up a storm. Tyler and Sarah are doing great in school and Tyler's team won the league championship this year.

These years are passing much too quickly for my liking. Both children speak often of their Grandmother Diana who is an Angel and they love you very much. After 19 years, I find it is no easier to deal with your passing. The wound is just as raw and painful as it was on the day the angels took you from me. Losing you left such immeasurable sorrow and an aching emptiness in my life that can never be filled.

If these words are familiar, it is because you have heard them before. The Apostle Paul said many things about love, and I believe our love mirrors Paul's teachings in that it has truly endured all things and it absolutely has never failed. It is knowing that our love is as real and powerful today as it was when you were by my side that sustains me though every day of my life. Dearest Diana, you are and will forever by my precious Wife, my one true love, and my soul mate. Rest in peace, Sweetheart.
